Before you Begin: 

• This can be used by anyone in the community who is attempting to decide if a person under the age of 25  should be connected for housing or other supports.

• Young people are encouraged to use this tool themselves  

• None of the information entered for the assessment is captured or stored.

• This tool will lead you through the conversation or self-discovery of their/your options for available supports, services, and programs depending on information about their/your situation. 

• At the end, there will be a referral portion, which is optional.
  Only the information that you state from that point on,
  and explicitly send, would be shared.
